Clinical Studies With a New Rifamycin Derivative

Howard D. Cohn M.D.1

1 CIBA Pharmaceutical Co., Summit, N.J. 07901.

Human pharmacodynamic studies were carried out with rifampin, the N-methylpiperazine derivative of rifamycin SV, a new semi-synthetic antibiotic belonging to the rifamycin family. Following the replication of an apparent mild, transient, unconjugated hyperbilirubinemia in normal volunteers, a series of studies was initiated in order to elicit the possible cause. No completely satisfactory explanation exists. The presence of a combined causation is not ruled out, not unlike that reported previously with novobiocin. A classification of sites where any drug might potentially interfere with the bilirubin " cycle" has also been presented. Finally, it should be pointed out that it is unlikely that these miniscule elevations have any clinical significane.
