1 Experimental Therapeutics Unit, Department of Medicine, University of Oklahoma Medical Center; the Veterans Administration hospital, Oklahoma City, Okla.

Ten patients with severe emphysema and chronic bronchitis were utilized in a double-blind, cross-over, placebo-controlled study with Cis-Paracoumaric acid. Clinical and subjective improvement, timed vital capacities, and 24-hour cough counts were recorded. Marked subjective improvement was obtained in four of ten subjects following cis-paracoumaric acid treatment, as compared to no instances of marked improvement in any of the patients following placebo treatment. No statistically significant difference was obtained in timed vital capacities or in 24-hour cough counts. It is suggested that because of the absence of toxicity and subjective improvement attendant on its use, cis-paracoumaric acid might be substituted for the more hazardous adrenocorticosteroid treatment in selected cases of pulmonary emphysema.
