Comparison of a Partially Synthetic Glycoside Strophanthidin-Arabinoside with Ouabain and Digitalis in Patients with Auricular Fibrillation

The semisynthetic glycoside, strophanthidin-arabinoside, was studied in eight patients with auricular fibrillation and moderate congestive failure. By intravenous injection this drug exerts the typical digitalis-like action in slowing the ventricular rate. Its potency and curve of action are similar to ouabain. The effect appears almost immediately after injection and develops fully in about an hour or less. The action persists for about ten hours and disappears in 24 hours or somewhat sooner. Absorption after oral administration is negligible. The curve of action is practically identical with that of ouabain. The results indicate that strophanthidin-arabinoside can serve as a substitute for ouabain for rapid digitalization by the intravenous route.
